Safe OTC Medications in Pregnancy

During pregnancy, even everyday symptoms like headaches, allergies, or nausea can feel more complicated—especially when you’re unsure which treatments are safe. This guide outlines OTC medications safe in pregnancy, categorized by common conditions, to help you find relief while protecting your health and your baby’s development.

The information below is based on recommendations from ACOG, AAFP, and UpToDate, and includes medications generally considered safe for use during pregnancy. Always consult your healthcare provider before starting any new medication, even if it’s available over the counter.

Tip: Choose medications with single active ingredients and avoid NSAIDs like ibuprofen, Aleve, Motrin, and Naprosyn, unless directed otherwise by your doctor.

Acne:

  • Topical benzoyl peroxide
  • Topical azelaic acid
  • Topical salicylic acid
  • Topical Glycolic acid

Allergies:

  • Cetirizine (Zyrtec)
  • Loratadine (Claritin)
  • Diphenhydramine (Benadryl)
  • Chlorpheniramine (Chlor-tabs)
  • Dexchlorpheniramine (Polaramine)
  • Cromolyn Sodium nasal spray (NasalCrom)
  • Fluticasone nasal spray (Flonase)
  • Budesonide nasal spray (Rhinocort)

Constipation:

  • Metamucil
  • Miralax
  • Magnesium hydroxide (Milk of Magnesia)
  • Lactulose
  • Bisacodyl (dulcolax)
  • Docusate Sodium (colace)

Cough/sore throat:

  • Dextromethorphan (Robitussin, Delsym)
  • Cepacol lozenges
  • Chloraseptic spray

Difficulty Sleeping:

  • Doxylamine (Unisom)
  • Diphenhydramine (Benadryl)
  • Magnesium 400mg nightly

Fever:

  • Tylenol

Headache:

  • Tylenol (follow dosing instructions on the bottle)
  • Magnesium oxide 400-800mg daily (preventative)
  • Riboflavin 400mg daily (preventative)

Heartburn:

  • Calcium carbonate (Tums)
  • H2 antihistamines (Pepcid)
  • Proton Pump Inhibitors (Omeprazole)

Hemorrhoids:

  • 1% hydrocortisone topical

Nasal congestion:

  • Nasal strips
  • Vicks vaporub
  • Eucalyptus or menthol lozenges
  • Cetirizine (Zyrtec)
  • Loratadine (claritin)
  • Cromolyn Sodium nasal spray (Nasalcrom)
  • Saline nasal spray
  • Pseudoephedrine, phenylephrine (Sudafed)*

*Note: Not safe during the first trimester or for those with chronic hypertension, gestational hypertension, or pre-eclampsia.

Nausea:

  • Vitamin B6 (pyridoxine)
  • Doxylamine (Unisom)
  • Diphenhydramine (benadryl)
  • Ginger, mint

Rashes:

  • Hydrocortisone cream 0.5% or 1%
  • Topical Benadryl cream
  • Clotrimazole and Miconazole cream

Restless Legs:

  • Magnesium oxide or Magnesium glycinate 400mg daily

Yeast Infection:

  • Miconazole (Monistat) 7 day
  • Clotrimazole 7 day
