New Patients
Thank you for choosing a WHCMA physician for your comprehensive and caring gynecologic and obstetrical care. If you have pertinent previous medical records, we kindly ask you to have copies of your records faxed to our office in advance of your appointment. Ideally, 48 hours beforehand would give your new physician ample time to review your records.
If needed, an Authorization for the Disclosure of PHI (record release) form may be downloaded here. Please complete all sections, sign and drop-off, fax or mail to the office from which you are transferring.
Transfers
If you are scheduled for a new appointment at Women’s Health you must have your previous medical records sent to our office 48 hrs prior to the day of your appointment. This will allow your new physician enough time to review your records. Simply download the record release (disclosure of information), complete, sign, and mail to the office from which you are transferring.
Patients transferring their obstetrical care to Women’s Health of Central Massachusetts will not be scheduled for an appointment until their medical records have been received and reviewed by our providers. Once the review is complete, our team will contact you to arrange your visit.
Cancellations
In the event you need to cancel or change an appointment, we ask that you please notify us at least 24 hours in advance. This will allow us to offer the time we have set-aside for your appointment to another patient.
