Our Online Patient Portal Makes it Easy to Stay in Touch with Us
When you become a patient of Women’s Health, you have the option of using our online patient portal. This is a secure website that gives you 24-hour access to your health information from anywhere you have an Internet connection. Using a secure user name and password, you can:
- Review your medical record
- Get results of lab tests or imaging exams
- Schedule non-urgent appointments
- Request prescription refills
- Send your doctor a confidential email

New-Patient Forms: Save Time by Completing Them Before Your First Visit
Ah, paperwork! It’s a necessary part of ensuring that we have accurate, up-to-date information about you. To save time, you can download, print and complete these forms before you come to Women’s Health for your first appointment:
- Contact Information Update & Patient Portal Activation Form – Essential so we know how to reach you, and you can have online access through our patient portal
- Notice of Privacy Practices (HIPAA) – This document is informational only; nothing to fill out or sign
- Patient Registration Form – Please complete the entire form and sign the Assignment of Benefits section (authorizing us to bill your insurance) as well as the Acknowledgment of Receipt of Privacy Practices section
- WHCMA Authorization for the Disclosure of PHI (personal health information), also known as a Medical Record Release – This form can be used to request records from other providers to be released to us, or for us to release records to another provider
- Obstetrical Care Form & Prenatal Genetic Questionnaire – For obstetrics patients only
